In order to help our school streamline our contact tracing procedures, we are asking that parents provide information regarding their students' vaccination status.
Please take a moment to complete this form, if you feel comfortable doing so. Unless we can confirm your students' vaccination status through ImmuNet, we will need to assume that students for whom a response has not been submitted are unvaccinated.
Please submit one form per student. At this time, we only need the information for the student(s) who attend Longfellow Elementary School.
Parents/guardians should keep their children home if they are experiencing symptom(s) associated with any communicable disease, including COVID-19. In addition, parents/guardians are to perform the following COVID-19 health check every day before the student leaves home.
A child should NOT come to school with any of these symptoms:
Fever of 100.4 degrees or higher
Sore throat
Cough
Difficulty breathing
Diarrhea or vomiting
New onset of severe headache (especially with fever), or
New loss of taste or smell
For persons with chronic conditions such as asthma, the symptoms should represent a change from baseline. Children exhibiting symptoms of COVID-19 may not attend school.

Dear Longfellow Families,
This Winter, the Howard County Public School System (HCPSS) will continue to use Synergy Conference Management to schedule Virtual Parent-Teacher Conferences on February 10th and 11th. Although parents and guardians are always welcome to communicate with teachers regarding student progress, February 10th and 11th are designated Parent-Teacher Conference days where parents are encouraged to meet with your student’s mathematics teacher. Beginning on January 28, 2022 at 8:00 am, through February 4, 2022 at 4:00 pm, you can log on to HCPSS Connect to access Synergy Conference Management (Direction can be accessed here). You can then choose your desired conference time(s) based on each teacher’s availability. For parents without Internet access, please contact our front office staff at 410-313-6879 for assistance in scheduling conferences. *Calls coming in after 12:00 pm on Wednesday, February 9th will require scheduling a conference after February 11th.
Tips for Scheduling Conferences:
Parent-Teacher Conferences will be scheduled in 15-minute increments. To allow transition time from one Google Meet to the next, consider staggering your appointments to allow for full 15-minute conferences.
Parents are encouraged to sign up with their student’s mathematics teachers. You can also sign up with other teachers based on their availability in Synergy Conference Management.
Related Arts Information
In order to give priority to parents with specific concerns about a related arts class, please see your options for conferencing with related arts teachers.
Option 1 - Come to a related arts team office hours session to get a progress update from all related arts teachers in a single Meet. A virtual waiting room will be used to prevent overlap between families.
