DESCRIPTION: The consent form advises the client (parent or adult student) of counseling policies, state and federal laws, and clients’ rights.

While parental (or adult student) consent to the IEP is an indication that the parent agrees to services, consent paperwork is necessary as it is documentation that the provider communicated areas such as: limits of confidentiality, consent for in-person services/telehealth services, including information regarding the student’s rights, risks, and responsibilities while receiving RS Counseling services. 

FREQUENCY: Consent should be obtained when a student is assigned to your caseload, and on a yearly basis thereafter.
