DESCRIPTION: Case Notes are legal documents which may be viewed by judges, attorneys, clients, etc.  They provide a measure of protection and substantiate compliance with auditors. Case Notes follow the G-I-R-P format:  Goal(s), Intervention(s), Response(s), and Plan. 

FREQUENCY: Every single in-person, face-to-face encounter with the student/parent/etc.; and/or every Telehealth service delivery (real time two-way video conferencing or a telephone session)
