Forms for students & parents

before you apply

Before you can apply for a Work-Based Learning opportunity, you must first get permission from a parent or guardian to interview. We love to celebrate success, so we ask parents and guardians to complete the media release form as well.

You can get the WBL permission and media release forms from my office at Bonds, your counselor at Bonds or your high school, or your instructor. You can also download them by clicking on the pictures below. If you have any questions about the forms, you can email me at mbdill@greenville.k12.sc.us or call my office at 864.355.1093.

You can return your forms the following ways:

• Scan the completed forms and email them to me at the email address above

• Return them to your school counselor and ask him/her to send them to me

• Deliver them in person to my office at Bonds (I like to see your faces!)

We try to place our students in businesses with safe, supportive, and educational environments. Student feedback is a critical part of the Work-Based Learning process. Please complete and return the work site evaluation form to Michelle Willis. You can pick up a form at my office or download it below.

If you have an immediate concern about any Work-Based Learning experience, please do not wait to report that information. Instead, contact Mr. Dill immediately.
