ClASS SELECTION

Instructions for ALL Students

ALL students planning to attend Churchill Jr. High for the 2025-2026 school year have two options for submitting a class selection request for the grade they will be entering. Students may:

A. Submit the Online Class Selection Form using the appropriate link below, OR

B. Complete and return a Class Selection Card to the Churchill Counseling Center. 

Both options are available below, however, students need only submit one - the Online Class Selection Form or Class Selection Card - NOT BOTH. Steps for completing these are as follows: 

1. WITH YOUR PARENT, use the Online Class Selection Form or Class Selection Card included below to request the classes you want to take next year.  Please note that a QR Code is provided at the top right corner of the Class Selection Card. Scanning this code links students to the Online Class Selection Form, ensuring that both options for submitting class a selection request are readily accessible. 

2. Be sure to include all student information requested at the top of the Online Class Selection Form or Class Selection Card.  

3. Submit your Online Class Selection Form or Class Selection Card to the Churchill Counseling Center.

4. Churchill school counselors will input your Class Selection Requests into the Granite School District Focus Information System.

5. Contact your Churchill School Counselor with questions and or concerns.
