Students may receive at school when the medication is medically necessary for health or learning and must be taken during the school day. All medication, whether prescription or over-the-counter, must be prescribed by a licensed healthcare provider.
School staff may administer medication to students only when the Medication Administration Request Form or Emergency Action Plan is completed by the parent and healthcare provider and present at the school. The Medication Administration Form is available to be printed below, and Emergency Action Plans can be found and printed under the Health Conditions tab above. Pre-printed forms are also available for pickup from the school.
No prescription medication will be given by school staff unless it is in a pharmacy-labeled container with the student's name, name of medication, date prescription was filled, and administration directions clearly marked. Over-the-counter (OTC) medications should be in new and unopened containers, unexpired, and labeled with the student's name and birthdate.
A new medication administration form and/or emergency action plan must be completed each school year, as well as anytime there is a change in medication or medication dosing instructions. At the beginning of each school year, or when a new medication is prescribed, an adult will need to bring the medicine along with the completed form(s) to the school. The school cannot give medicine to your child without these form(s).
If medications are prescribed or indicated on the student's emergency action plan for the management of a medical condition, the parent/guardian is responsible for supplying these medications to the school within the first 10 days of school beginning. In the the event no medication is available at the time the emergency occurs, the school will call 911.
Parents or guardians are responsible for monitoring medication expiration dates and replacing medication when expired.
Self-Carry Medications:
Only inhalers, epinephrine auto-injectors (EpiPen), or diabetes supplies/medications may be self-carried and administered by a student, and only when the student is deemed competent by the prescribing physician and the school nurse.
Disposal of Unused Medication:
All unused, discontinued, or expired medication must be returned to the parent or guardian or disposed of in accordance with the Graham County School Health Policies. Parents will be contacted to pick up any medication remaining at school. All medication not picked up on the last day of school will be disposed of according to this policy.
