Have had a physical exam in the last year or upload proof of an upcoming appointment (appointment card, email or screenshot of text).

You will need to upload proof from your doctor. If you need a doctor, we can help you find one. DO NOT UPLOAD ANY MEDICAL RECORDS - ONLY A STATEMENT THAT YOU HAVE BEEN SEEN, SIMILAR TO A SCHOOL NOTE.

Have you seen a dentist in the last year or upload proof of an upcoming appointment (appointment card, email or screenshot of text).

You will need to upload proof of your dental visit. If you need a dentist, we can help you find one. DO NOT UPLOAD ANY MEDICAL RECORDS - ONLY A STATEMENT THAT YOU HAVE BEEN SEEN, SIMILAR TO A SCHOOL NOTE.

Upload a copy of your vaccination record including proof of HPV*

*If you have opted out of vaccines, you will need to read the information sheets and answer questions for each vaccine you have not received so we know you are fully informed. 

You can get your immunization record from your doctor, parents or school nurse.

Keep a Food Diary tracking fruit/veggies and water intake for a week. Download a copy of the Food Diary here.

You will need to upload the food diary to your google form.

Volunteer with a local non-profit or organization or participate in a health promotion event.

Examples:  participating on any athletic team, or marching band, volunteering at a community nonprofit like Meals on Wheels, East Texas Food bank, a run/walk club, or a local charity 5K fun run.  You choose!  

Watch and Read 5 online training videos or articles and complete the questions.
