➢Each and every person stepping foot onto the field at Ford Field must be listed on a fully executed form (students, volunteers, staff, etc.)
➢ ONLY PERSONS OF LEGAL AGE (18) may fill out these forms.
➢ The forms MUST BE COMPLETED IN INK and MUST NOT BE ALTERED IN ANY WAY.
