Dear Parents:
This letter serves to notify you that Dacula Middle School will conduct our annual screening for scoliosis on 11/01/2023. The screening is done yearly for students in 6th and 8th grades as required by Georgia State Law and mandated by the Georgia Department of Public Health. The screening is done to identify students who may have scoliosis, an abnormal curvature of the spine. Scoliosis affects one to two percent of the adolescent population. If the condition is detected early and appropriately treated, progressive spine deformity can be prevented.
The procedure for screening is a simple process in which the screener (nurse, physical education teacher or trained volunteer) looks at the child’s back, in the standing position and as they bend forward, for any possible abnormal curvature. Boys and girls are screened separately. Girls should wear a sports bra or bathing suit top that allows the screener to see their back.
In the case of a suspected curvature, you will be notified by the Department of Public Health and referred for secondary screening. This does not mean that your child has scoliosis only that further evaluation by a health care provider or the Department of Health is warranted.
If your child has already been screened by a health care provider or you do not want your child screened at school, you must submit the Department of Public Health Form 4400 linked below to the school no later than Friday, October 27, 2023 if you have not already submitted it to your child's homeroom teacher.
Sincerely,
Charisse Deramus
Assistant Principal

Georgia Parent Survey
The Georgia Parent Survey is designed to give parents an opportunity to share input with the Georgia Department of Education. It contains 24 questions. Parents may complete the survey using a personal computer, smartphone, or tablet. Parents that do not have access to the Internet, should contact their child’s school to arrange to complete the survey at the school.
The data parents provide by completing this survey will be used as part of the calculation of the School Climate Star Rating. Survey responses are anonymous and will be submitted directly to the Georgia Department of Education for analysis.
Individual data results from the Georgia Parent Survey will not be available to system or school personnel or the public and will not be posted online.
Please provide your voice for Gwinnett County Public Schools by clicking HERE and completing the survey.
Georgia Student Health Survey
To help improve our schools and those across the state, Gwinnett County Public Schools students in grades 3-12 will participate in the Georgia Student Health Survey. The GSHS is an anonymous and statewide survey instrument that covers topics such as school climate and safety, peer, and adult social support, bullying, mental health, substance abuse, and suicidal ideation.
The Student Survey will be administered for Dacula Middle students via the GCPS Student Portal during the week of November 13, 2023.
Parents may complete the Opt-Out process if they do not wish their student(s) to participate in the survey. Parents may use the Parent Portal to complete this process, or they may send a written request to the schools to opt their student(s) out of this survey. Please note that any parent/guardian wishing to opt his/her student out of the survey should complete the online form or inform the school in writing at least 72 business hours before the school has scheduled the administration of the survey to ensure that the student does not participate.
Click HERE to learn more about the GSHS survey, to review the GSHS Survey, and to learn about the opt-out process if you do not wish for your student to participate.
For safety reasons, morning car rider students should not exit their vehicles until the first bell rings at 8:55am. Students are not allowed to enter the building until that time as there is no staff supervision available.
Students needing to be dropped off prior to the morning bell must use the bus lanes and enter the cafeteria up until 8:35am. After that point, all traffic must go through and line up in the car rider lanes at the front of the building.
Thank you for your cooperation and attention to this important safety issue!
