Before you begin, please note:
The forms below are required for all students new to the WorkAbility Program.
Please complete the application in its entirety to ensure prompt processing.
Sign and date all forms were required.
Once completed, return the application to your teacher.
In addition to the completed application, students must provide:
A valid California ID
A signed Social Security Card
Note: If you do not have a California ID, your teacher will include proof of your enrollment in school.

Please note: If you are under the age of 18, the parent/guardian must sign where indicated. If the student is conserved, the Conservator will need to sign the application and include a copy of the conservatorship documents with the packet.
Along with your application, please ensure you complete and submit the following forms:
Parent/Student Consent Form: If you are under the age of 18 or if you are have a conservator, These forms must be signed by a parent or guardian to give consent for your participation in the program.
Work Ethic Agreement: This agreement outlines the expectations and responsibilities of students within the WorkAbility Program and is required to have the students signature.
