To enroll in the Department of Rehabilitation (DOR) program through the East Side Union High School District (ESUHSD) Adult Transition Program, students must meet certain eligibility requirements and follow a specific application process. Here's an overview of the enrollment process:
To be eligible for DOR services, a student must:
Have a physical or mental impairment that substantially impedes their ability to secure employment.
Require vocational rehabilitation services to prepare for, secure, retain, or regain employment consistent with their strengths, resources, priorities, concerns, abilities, interests, and informed choices.
Be able to benefit from DOR services in terms of an employment outcome in an integrated setting.
Complete the application: Fill out all required fields on the forms
Provide detailed information about your disability and how it impacts your daily activities, school, or current/future job
Submit the application:
Turn in the completed forms to your student's teacher.
Your teacher will forward the application to the appropriate DOR service coordinator
Await further instructions:
The DOR will review your application and contact you for an intake appointment
Be prepared to provide additional documentation about your disability and any benefits you receive
Remember that even if you are found eligible, you may be placed on a waiting list depending on the availability of resources and the severity of your disability. The DOR uses an Order of Selection process to ensure those with the most significant disabilities are served first when funding is limited.
