You will be asked to complete an Immunization Log entry for each child enrolled in Head Start at your site.  The FSWA will provide you with the Immunization Log, listing each Head Start child's name. You will put the status at the beginning of enrollment. We follow your ISD's guidelines for immunization requirements.  A new Immunization Log will be provided for each student enrolling after the first day of school for you to complete the status. Closer to the end of the year, the FSWA will request that you complete the End of Enrollment section on each Immunization Log.  The FSWA can also assist you in getting updated immunization records if needed.
