Explanation of Head Start Child School Nurse Supervision Requirements:
Complete the Active Supervision and Positive Guidance Form with FSWA twice a year.
Head Start staff or a designated adult must escort the Head Start child to and from class for every visit the child makes to the office or the nurse.
The school nurse or designated adult must maintain continuous supervision while the child is in the nurse’s office
The Head Start student cannot be taken from the nurse’s office to another department or contractor without the Head Start teacher's permission and knowledge.
