We respectfully share with you QR Code for Generali LOA Request 2024. By utilizing and submitting this form, you are granting authorization to the Social Welfare Employees Association of the Philippines (SWEAP) to utilize and share the provided information. This includes utilizing the data to offer relevant health services and benefits through HMO/HCSP, Generali, as well as with the relevant clinic, hospital, and medical professionals.
Instruction:
1. Please submit your request for outpatient healthcare services at least two to three days before the intended date of availing services. This timeframe is particularly important as the SWEAP and Generali teams are currently in a transition period.
2. Before requesting a Letter of Approval (LOA), it is advisable to verify if your name is on the list of eligible individuals through your respective SWEAP Chapter Focals.
3. When filling out the Google Form, ensure to specify the chief complaints or reasons for consultation. Additionally, attach any doctor’s referral or request if available.
4. Include relevant details such as the name and address of the hospital, as well as the attending doctor, if known.
5. If any section of the Google Form is not applicable to your situation, please indicate "N/A" accordingly.
6. You are encouraged to share the QR code and link of the Google Form with other eligible individuals who may need to avail of outpatient healthcare services.

We also have CSR team with their assigned chapter to ensure seamless communication. In the event that one of the CSRs is unavailable due to unforeseen circumstances, the other CSR will assume responsibility for the unavailable CSR.
