The Re-Entry Action Plan (REAP) is a document that outlines how you would apply the concepts and competencies you learned during the learning session.
It is a document required on all attendees by the User Support Division (USD) as part of its conduct of learning and development programs.
An Authority to Attend is a document indicating that your Head of Office gives their permission and support in your participation in USD’s events.
It is also a document required by the Central Office’s Accounting Division to process any necessary disbursements related to training, especially if said events were conducted online and through external partners.
The REAP enables you to assure that all the learning and insights you gained in the program shall be significantly applied to your respective workplace. It is a proactive process that facilitates discussions and agreements between you and supervisor on the focus of the strategy of the REAP.
It promotes the building of a commitment between you as an attendee and your supervisor, who approved your attendance at the learning event, to address identified development goals relevant to the practice of your duties.

To edit the document, you must download it first to your device; in Microsoft Word Online, select More > Download.
You must have a Microsoft 365 account linked to your DepEd email address to download the document.
Apart from personal and employment details, fill out the following items in the REAP:
Objectives - what you want to achieve for each activity; follow the SMART method and Bloom’s taxonomy
Activities/Strategies - what events, approaches, or methods you would want to adopt to fulfill the objective/s stated
Timeframe - state the time when you know you would be able to successfully implement the activity and fulfill the objective
Participants/Beneficiaries - identify who you target to perform the activities with
Resources Needed - state what materials or support you require to enable the fulfillment of the objective; item is optional
Expected Output/Outcomes - what you want to achieve in the short term or throughout the conduct of the activity
Remarks - any information that would not be covered in the previous items
The document to be submitted must be a Portable Document Format file (*.PDF).
You can insert additional rows when you need to.
The document is pre-formatted. Other than adding rows to the table, filling in required details, and attaching signatures, you are not allowed to change the document’s layout or format.
The Supervisor in the REAP is someone in authority that directly supervises the personnel and/or their school or office.
Any one of the following is authorized to sign the REAP, provided they are the attendee’s immediate supervisor:

You can use electronic or digital signatures on the document.
You should submit one (1) REAP per certification course.
Follow this naming convention: OriginalFileName_[LASTNAME]_[FIRSTNAME].
For example: USDREAPforMOS_DELACRUZ_JUAN

For the 2022 MOS for Admins program: All REAPs, Authorities to Attend, and Microsoft acknowledgment letters must be sent through this link: bit.ly/2022MOSRequirements by 23 June 2022, 12:00 PM.
No. Unsigned or improperly filled-out documents are considered invalid.
As previously discussed, the documents are required for you to proceed further with attending your learning event.
If you fail to submit any one specific document required by the activity, you will not be permitted to attend the learning event any further.
