Schedule Change Form

Schedule changes can be made during the first few weeks of school.  All schedule changes need to be made through the school counselor's office utilizing the form below.  It can be filled out electronically and returned to Ms. Jaros in the school counselor's office at jjaros@deerlakes.net.  Students are to follow their current schedule until a revised schedule is sent to the child and his/her teachers by the school counselor.
