The Dentist is coming to DMS!!!

Greater Family Health will be providing dental exams to DMS on November 17th and 18th. This counts for the 6th and 9th grade dental exam. Please give your consent by completing this form and returning it to the Nurses' Office by November 1st.

 6th Grade Dental Exams are due by May 15th. 

Sports Physical 

If your student plans to try-out for a sport next year, please do not delay. complete a sports physical over the summer. Students are not allowed to try-out for a sport without the completed sport physical. 

Medication at School

If you want your student to receive medication during the school year, please complete the d300 medication form. There is a parent portion and a doctor portion. Medication needs to be dropped off by an adult, with the completed form. This includes: Advil, Tylenol, any otc medication.

Medication Drop Off

ALL medications that students will take at school need to brought to school by a parent or an adult. Students are not permitted to carry medication with them. If you have any questions, please call the nurse's office.
