Annual Open Enrollment
District 300's Open Enrollment is held annually between October 15th - November 15th
All benefit-eligible employees must complete Open Enrollment to change/continue/waive insurance elections that would begin January 1st. Enrollment is completed through American Fidelity's portal, AF Enroll. Enrollment information and details are emailed to employees in October and posted in the Announcements hyperlink below.
You are able to make changes to your insurance coverage outside of Open Enrollment if you have a qualifying life event, click here.

Review your current insurance elections in Lawson Employee Self-Service (tile in Rapid Identity)
Click on Menu > Bookmarks > Employee Self-Service > Benefits > Current Benefits

Completing Open Enrollment (October 15th - November 15th)
Open Enrollment is completed in American Fidelity's portal AFEnroll. You have two options to complete it:
Schedule an appointment with an American Fidelity Representative, click on the "AF Appointment" link below
Virtual appointments are available October 15th - October 31st and in person will be held at the District Office November 1st - November 15th

Complete on your own using the below "AF Self-Enroll Instructions" available in English or Spanish
You will be prompted to set a PIN (the one from last year has been reset) and set up verification using a Two-Factor Authentication Code that is sent to your D300 email or phone number in Infor Lawson Employee Self-Service
Please write your PIN down you will need it to confirm your insurance elections at the end of your enrollment.
To avoid missing Open Enrollment we HIGHLY recommend you sign in and complete your elections before the 15th of November.
Employees can enroll or make changes to their insurance elections through November 15th. Insurance elections made during Open Enrollment will begin on January 1, 2024.

Health - Blue Advantage HMO Participants - To receive an insurance card you will need to select an HMO Medical Group. To locate an HMO Medical Group and/or add your HMO Medical Group to our system please use the following instructions, click here.
Health - Blue Choice Options HSA Participants - To receive your Board HSA Contribution you will need to open an HSA Account with First American Bank, for instructions click here.
Dental - Delta Dental DHMO (DeltaCare) Participants - To receive an insurance card you will need to select a provider and provide Delta Denta with a Facility ID number, click here.
VOYA Supplemental Life Insurance (Enroll/Change/Cancel)

VOYA Group Life Insurance is paid for by the Board. For more information on this coverage click here.
VOYA Supplemental LIfe
Employees enrolled in VOYA Group Life insurance are eligible to participate in Supplemental Life Insurance coverage. This coverage is optional and is paid for by employees through payroll deductions. You can view your current coverage in Infor Lawson Employee Self-Service under Current Benefits.
If you are interested in enrolling in, increasing, decreasing, or canceling supplemental insurance coverage during Open Enrollment please follow the below steps.

Complete a *VOYA Enrollment Form, click here, and email it HR.Benefits@D300.org or fax it to 847-551-8393 on/before November 15th
If you are adding coverage for a *Spouse, you must be enrolled in Supplemental Coverage. Your Spouses coverage must equal 50% or less of than your coverage. Please email a certified copy of your marriage certificate to HR.Benefits@D300.org
If you are adding coverage for *Child(ren), you must be enrolled in Supplemental Coverage. Please email a certified copy of your children's birth certificate to HR.Benefits@D300.org

If you are enrolling in or increasing your current VOYA Supplemental Coverage you will also need to complete an Evidence of Insurability, click here, and submit it directly to VOYA (see instructions on form) by November 30th. Current Beneficiary - to view your current life insurance beneficiary click on Document Self Service in Rapid Identity > click on My Employee Documents
Add/Update Beneficiary - complete the VOYA Beneficiary Form, click here, and email it to HR.Benefits@D300.org
Dependent Verification
Employees who add a dependent or were flagged as missing documents for their dependent(s) will have their Open Enrollment election(s) placed on hold until certified documents (issued by the county, ex. marriage certificate for Spouse and/or birth certificate for dependent child(ren) are provided.
You will receive an email from American Fidelity requesting you upload the requested documents into their secure server or you can provide it to our office using one of the below methods. Dependents must be verified as eligible before their elections can be sent to the insurance provider(s).

Once our office receives your document(s) we will update your dependent(s) in our system, confirm receipt in AFEnroll, and upload the document(s) into Document Self-Service.
Eligible Dependents include:
Married spouse and civil union partner as defined under the Illinois Religious Freedom and Civil Union Act
Natural children, stepchildren, adopted children (including children in employees' custody under an interim court order of adoption or placed with the employee for adoption or placed with the employee for adoption vesting temporary care) or children for whom the employee is legal guardian under age 26
Unmarried children to age 30 residing within Illinois, having served as an active or reserve member of the Armed Forces and released or discharged with other than a dishonorable discharge
Children incapable of self-sustaining employment and dependent upon the employee or other care providers for lifetime care and supervision because of a handicapped condition occurring prior to reaching the limiting age regardless of age
