New Hire Enrollment
Welcome to District 300!
As a *new hire, you have 30 days from your start date to complete your New Hire Benefit Enrollment.
Steps 1 and 2 can be completed prior to starting your new position.
Steps 3 and 4 can be completed on/after your start date.
Your insurance coverage will begin
The first day of the month following your start date
Example: If your start date is 8/11, your insurance coverage will begin 9/1.
The first day of the month if your Start Date is the 1st of the month.
Example: if your Start Date is 9/1, coverage would begin 9/1.

Step 2 - Complete and Submit VOYA Life Insurance Enrollment Form
District 300 offers Group Term Life Insurance that is paid for by the Board. The amount of coverage is listed in your Labor Agreement, for more information click here. During your enrollment period (30 days from the start date) you are eligible to enroll in Supplemental Life Insurance paid for by you. For more information and rates click here.
Please complete the below Voya Enrollment Form within 30 days of your start date.
VOYA Enrollment Form - click here, and upload it into the following Google Form, click here (you will need to have District 300 email access to upload the form) or email it to HR.Benefits@D300.org. For instructions on how to complete this form, click here
Step 3 - Complete New Hire Enrollment Online
On your start date, you will have access to Infor Lawson Employee Self-Service (tile in Rapid Identity) to complete your New Hire Enrollment. You have 30 calendar days from your start date to complete this.

Don't forget to print a copy of your elections for your records.
Step 4 - Provide Certified Documents for Dependents
If you are enrolling a spouse or dependent children please provide the following certified documents (issued by the county) if you are enrolling them onto your insurance plan(s).
Spouse - marriage certificate
Dependent Children - birth certificate or guardianship/adoption paperwork
There are three ways to provide the above document(s).

Scan a copy (picture with phone or scanner) and email to HR.Benefits@D300.org
Drop a copy off in Central Office's locked dropbox, attention HR Benefits (2550 Harnish Drive, Algonquin, IL 60102
Step 5 - HMO and HSA Participants
If you enrolled in the following plans you have additional steps to complete.
Health - Blue Advantage HMO Participants - To receive an insurance card you will need to select an HMO Medical Group. To locate an HMO Medical Group and/or add your HMO Medical Group to our system please use the following instructions, click here.
Health - Blue Choice Options HSA Participants - To receive your Board HSA Contribution you will need to open an HSA Account with First American Bank, for instructions click here.
Dental - Delta Dental DHMO (DeltaCare) Participants - To receive an insurance card you will need to select a provider and provide Delta Denta with a Facility ID number, click here.
What's next?
Insurance elections are processed and sent to the insurance providers (health, dental, vision) once a week via an electronic feed on Monday. The sooner you complete your online enrollment the faster you will receive your health and dental insurance cards, you will not receive a vision insurance card.
Insurance premiums are deducted semi-monthly, if you miss a premium deduction notice will be sent to you with the amount due. To avoid a double deduction the following pay period complete New Hire Enrollment as soon as possible.
If you did not print your elections, you are able to view your elections online once they are processed (check a week after completing your elections) in Infor Lawson Under Employee Self Service > Benefits > Current Benefits
Your elections made as a new hire will end on December 31st. To continue your coverage please complete Open Enrollment held October 15th – November 15th to continue your insurance coverage on January 1st. More information on Open Enrollment will be sent at the beginning of October.
Changes can be made to your insurance coverage outside of Open Enrollment if you have a qualifying life event, click here.
New Hires in October and November
As a New Hire in the month of October and November, you will need to complete two insurance benefit enrollments.
Step 1: New Hire Enrollment complete within 30 days of your start date. This coverage begins the first day of the month following your start date.

If you start in October, your coverage will begin on November 1st. Exception if you start on October 1st your coverage begins October 1st
If you start in November, your coverage will begin on December 1st. Exception if you start on November 1st your coverage will begin on November 1st
Step 2: Open Enrollment - complete between October 15th - November 15th. This coverage begins on January 1st

If you do not complete New Hire Enrollment your next opportunity to enroll in District 300 benefits will be during Open Enrollment October 15th - November 15th for coverage that begins January 1st.
If you complete your New Hire Enrollment after Open Enrollment ends your New Hire Enrollment elections will roll over into the next year, except Flexible Spending.
Please note: If you miss both New Hire Enrollment and Open Enrollment you will not be able to enroll in District 300 insurance plans until Open Enrollment next year unless you have a qualifying life event, click here.
