Medication Forms

Below you will find links to the medication forms needed for band camp and throughout the season. There are three forms:

Non-Prescription Medications (Updated July 2025)

There are three forms (two are combined into one document here) in total to allow students to receive Over the Counter medication while traveling with the Crestview Band Program. It is recommended that this be completed for ALL students so they can receive over the counter medications if needed at band events.

Students and Parents/Guardians should first complete the OVERNIGHT FIELD TRIP STUDENT HEALTH FORM. This form covers basic pain medication, nausia medication, and sunscreen. The NON-PRESCRIPTION MEDICATION FORM covers all other medications that are not prescribed by a doctor and those over the counter medications provided by the Music Boosters. This form only needs to be completed once. 

Prescription Medications & Inhalers

Student that must take prescription medications while at band camp (or at other events) must complete both pages of the PRESCRIPTION MEDICATION FORM for each prescription. There is no digital component of this form.

If a student needs to use an inhaler, they must complet the INHALER MEDICATION FORM. You DO NOT need to complete the Prescription Medication Form for inhalers.
