Roles and Responsibilities

Intervention Specialist / School Team


  • Transition Meeting with Parents/Students – and appropriate agencies

  • Review/Update career interests and information on post-secondary goals

  • Complete District Transition Assessment if appropriate

  • Identify appropriate community/state agencies – Review services

  • Discuss state issued Identification

  • Consider travel training

  • Consider Transitional Services for Senior Year

  • Introduce Project SEARCH / ICAN

  • Consider Project LIFE

  • Review appropriate apprenticeship opportunities

  • Concrete top 2 options for Post-Secondary (Enrollment, Employment, Enlistment)

  • Explore options – Field Trips

  • Career Fairs

  • Introduce the necessity of Financial Banking (personal)

  • Consider Financial and Future planning for independent needs

  • IF ENLISTMENT – ASVAB should be discussed

  • Make referral for district Career Assessment if appropriate

Parent / Guardian


  • Attend and be involved in IEP meeting

  • Continue collaboration with OOD and DDS

  • Attend Parent/Teacher meetings

  • Discuss career pathways with your child

  • Help update the backwards transition plan

  • Attend transition information sessions

  • Monitor your child’s grades and attendance

  • Make sure your child takes the ACT or other entrance requirements for post-secondary career path

  • Help visit post-secondary programs

Student


  • Attend and be involved in IEP meeting

  • Invite OOD and DDS to your IEP meeting (if appropriate)

  • Complete Naviance activities

  • Meet with Guidance Counselor and Intervention Specialist to discuss course options

  • Complete community work experience if appropriate

  • Take ACT and other entrance requirements for your post-secondary path

  • Visit colleges or trade schools

  • Research requirements for post-secondary career option

  • Job shadow in a career interest area

  • Update backwards planning form

  • Attend transition information session
