Forms Used for Identification

The following forms are used to help track qualifications for students referred to the AIG program.  Once student data has been recorded on the form, the AIG Coordinator will meet with the CCS Council for Gifted Education.  The team will look at the data to determine if the student meets the qualifications required for identification.  These forms are internal to CCS and parents do not need to worry about printing any or filling any in.
