The Behavior Referral Form allows school staff members to complete an online referral for a student behavior event and submit it to a staff member responsible for managing behavior for further action.

Public School Works
Teachers are NOT required to complete the Behavior PSW assessment. All School Administrators responsible for resolving and finalizing Behavior Referrals must complete a Public School Works assessment at the beginning of every school year.
