Please note that if there is any cost to use a product you should not use this form. Instead submit your request as a budget item to your building principal.
Once a service is submitted it goes through a review process. If deemed appropriate for use by the district, the vendor is sent a privacy agreement. Until this form is signed and returned, the item will appear on the "Review in Progress" list.
When the district is in receipt of the agreement, submitted items will be placed on the "Approved" list. The person who submitted the resource for review will be notified directly as well.
If the item submitted is either not approved by the district OR the vendor will not sign the district's agreement, the item will be placed on the "Not Approved List".
*NOTE: There is no standard duration for this process to be completed. It will be dependent on the length of time it takes for the vendor to sign the agreement. If the vendor does not respond to our request to sign the district's Data Privacy Agreement within 3 months, the item will be moved to the Not Approved list.
