New Student Enrollment

Step 1: Complete the CMS New Student Enrollment packet

• New Student Enrollment Forms can be found HERE

• The following documents are required for enrollment:

  • Student Enrollment Form

  • Proof of date of birth and legal name

  • Proof of Residency

  • Safe Schools Declaration

  • Current Immunization Record*

  • All children entering NC Public Schools  for the first time must submit proof of a Health Assessment.*

• *These documents are required upon student entry. A "30 calendar day grace period" for submission is granted for students not having required documentation.

Step 2: Contact Christine Cassino at christine.cassino@cms.k12.nc.us to submit your packet, or drop it off at Mallard Creek during school hours.

Step 3: Once your completed packet has been submitted and processed by Student Placement, you will be contacted by a school counselor to begin course selection.

*PLEASE NOTE: Families will be unable to select courses without the following:

• A completed new student enrollment packet that has been submited to Mallard Creek Student Services.

• A copy of the student's transcript (rising 10-12th), or report card (rising 9th). You can fax these documents to Christine Casssino at 980-343-1042 or email them to christine.cassino@cms.k12.nc.us
