This trip is Arizona state tax credit eligible. You can choose to make payments as a direct payment towards the trip or as a tax credit payment. Please specify with each payment if you want it as direct payment or as tax credit. If using tax credits please fill out the form below in full with each payment. As a change this year all payments will be made through the CJSD business office. If using tax credit donations you may also utilize the online payment system linked below. Tax credit donations are non-refundable. Direct payments are fully refundable until April 1st, -30% past April 2nd - April 24th and non-refundable after April 25th.
