REQUIRED REGISTRATION DOCUMENTS AND FORMS
This is a listing of the required forms for student registrations at Chariho Regional School District. Please use this listing as a guide to assure you have all necessary documents. Out of District CTC students, with an approved CTC application, may skip ITEM 1 but must provide the Approved CTC application with Registration documents.
ITEM 1: Locator Card: To obtain a Locator Card, bring the following documents to the Charlestown, Richmond or Hopkinton Town Hall:
Proof of Residency: Bring any of the following: RI driver’s license with current street address AND either a lease/mortgage statement, a tax bill, mail with current street address, vehicle registration.
NOTE: If you are moving into another person’s home, they must prove residency as above and sign a sworn statement.
Student’s Birth Certificate: If you are a single parent, legal custody papers (i.e. divorce papers, death certificate, guardianship papers from Family/Probate Court)
If the Chariho District has questions regarding the residency of any child, the Attendance Officer or a Police Officer will be dispatched to verify residency. If it is determined that a student’s residency in Chariho was claimed fraudulently and/or under false pretenses, responsible parents or guardians will be charged tuition for each day of enrollment in Chariho schools. Enrollment in Chariho schools will cease.
ITEM 2: Copy of Legal Guardianship (if applicable)
Court appointed custody papers, divorce papers, death certificate, or guardian papers from Family/Probate Court.
If a family is willing to allow an out-of-district student to live with them, they must file for Temporary Guardianship by filling out an affidavit acting in LOCO parentis. In LOCO Parentis Form
ITEM 3: Copy of Birth Certificate
ITEM 4: Immunization/Physical Exam: It is the policy of the Chariho Regional School District that: Every student who has not been previously enrolled in a school in Rhode Island must have a medical history and physical examination completed, signed, and submitted to the school nurse prior to entry into a school in the Chariho Regional School District. Evidence of immunizations must also be provided and must meet current State of Rhode Island immunization requirements. Additionally, Preschool/Kindergarten registrants must provide proof of lead and vision screenings. Immunization Requirements, RI School Physical Form, Dental Form. ITEM 5: A Transcript or copy of last report card and withdrawal grades. (When applicable)
ITEM 6: A Consent to Release Records Form needs to be filled out and signed.
ITEM 6: Copy of Parent/Guardian licenses front and back.
ITEM 7: Transportation and Bus Release for Grade K and 1 student
Out of District CTC students will receive communication from the CTC Director by June 1 with Transportation information.
Students attending out of District CTC programs in Exeter - West Greenwich, Narragansett, North Kingstown, South Kingstown or Westerly you must submit a Transportation request each year to Ocean State Transit at mpelchat@ridesta.com
Requests to Attend Chariho Schools
With the exception of applicants for career and technical programs, requests of students from other districts to pay tuition to attend Chariho schools must be approved by the Superintendent of Schools. In extenuating circumstances and with the prior approval of the Superintendent, students who are moving to one of the member towns may be allowed to attend Chariho schools without payment of tuition prior to October 1st if residency will be established by October 1st. Transportation will not be provided when requests are granted.
Student Residence Changes
If a student’s residence changes to an out-of-district location after completion of his/her junior year in high school, the student will be allowed to complete his/her senior year at Chariho Regional High School. If any current Chariho student’s residence changes to a location other than one of the member towns during the course of a semester, the student will be allowed to complete the current semester in the Chariho schools. Transportation will not be provided when these options are exercised. These guidelines will not apply if it is determined that a student’s residency in a member town was claimed fraudulently and/or under false pretenses.
