Badge ID Photo Guidelines
1. In color.
2. Taken within the past 6 months, showing current appearance.
3. Full face, front view with a plain white or off-white background.
4. Taken in normal street attire: Uniforms should not be worn in photographs except religious attire that is worn daily.
5. Do not wear a hat or headgear that obscures the hair or hair line.
6. If you normally wear a hearing device, wig or similar articles, they should be worn for your picture.
7. No eye glasses (even if you wear them regularly)
8. Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required).
9. Articles worn for religious or medical reasons should be accompanied by a statement to that effect

Building Access- Fill out Form for Card Access
Your CHA badge will allow you to access the the CMC office and all rooms therein. However, the CMC office is located within a larger office building not owned by CHA or CMC. This building is open from 8am-5pm, Monday-Friday. To enter the office building outside of these hours, you will need a building access key card. Please complete the form below in order to request a key card. Even if you will likely not be coming into the office outside of the 8am-5pm hours, we encourage all employees to have a key card in case an emergency arises or they forget something in the office.
