ANSWER:
1. SERVICE REQUEST FORM (SRF) - Click this link to download SRF - DOWNLOAD SRF
2. LICENSE TO OPERATE (LTO) - Must be valid, readable and colored.
ANSWER: SRF = Service Request Form.
ANSWER: To upload your documents simultaneously, compressed or zipped the documents to reduce the file size. (SRF/LTO).
ANSWER: The usual processing time is 3-5 business days upon request. (except weekends and holidays)

STEPS
1. Go to EDPMS website and LOG-IN your account.

2. Download and fill out the "Service Request Form (SRF)".

Note:
MAKE SURE THAT YOUR SRF IS SIGNED AND DATED.
WE ONLY ACCEPT SRF DATED 1-2 WEEKS FROM REQUEST AND READABLE/COLORED LTO.
3. In EDPMS website (see step 1), Click "Online Services" and choose "Service Request".
-at this point, make sure you are logged in.
4. Fill out the form with correct information, attach the compressed/zipped file (SRF, LTO) and click submit. (Check the image for the guide)

4.1. If you can't submit your request, try to re-select the "CATEGORY".
5. Save your reference number and wait for our email/text/call for any updates on your request.
