FREQUENTY ASKED QUESTIONS

WHAT ARE THE REQUIREMENTS?

ANSWER:
1. COMPANY REGISTRATION FORM (CRF) -Click this link to download CRF - DOWNLOAD CRF

2. SERVICE REQUEST FORM (SRF) - Click this link to download SRF - DOWNLOAD SRF

3. LICENSE TO OPERATE (LTO) - MUST BE VALID, READABLE AND COLORED.

PS. Kindly read and follow the steps provided below.

ANSWER:

SRF = Service Request Form
CRF = Company Registration Form

ANSWER: To upload your documents simultaneously, compressed or zipped the documents to reduce the file size. (CRF/SRF/LTO).

ANSWER: The usual processing time is 3-5 business days upon request. (except weekends and holidays)

REMINDER - READ EVERY STEP

STEPS

1. Go to EDPMS website
Click this link to go to the website - Electronic Drug Price Monitoring System

2. Download and accomplish both "Company Registration Form (CRF)" and "Service Request Form (SRF)".

NOTE:
MAKE SURE THAT YOUR DOCUMENTS ARE SIGNED AND DATED.
WE ONLY ACCEPT SRF & CRF DATED 1-2 WEEKS FROM REQUEST AND READABLE/COLORED LTO.

3. In the EDPMS website (see step 1), click "Online Services" and choose "Company Registration".

4. Fill out the form with correct information, attach the compressed or zipped file (CRF, SRF, LTO) and click submit. (Check the image for the guide)

5. Save your reference number and wait for our email/text/call for any updates on your request.
