Use this form to check out of the classroom for the restroom or any other hall pass.

Step 2: If approved, use the form to "check out" and of the class by filling in all questions.

Step 3: Acquire the Teacher's Pass and proceed to and from the location in a timely fashion.

Step 4: Upon returning, use the form again to make a new "check in" entry.
