If you have a school-aged child living with a parent/guardian within the boundaries of Bonita Unified and attending a private school, and believe there may be a learning or other type of disability, you can request a special education assessment through the Bonita Unified School District.

Please complete and sign both pages of the enrollment form, then bring those pages, along with proof of residence within Bonita Unified School District, to the Office of Student Services at 115 W Allen Ave, San Dimas, CA 91773. Once all documentation has been verified, the assessment process can be initiated.
