All work-related incidents need to be reported, regardless of their consequence or severity, including:

• Illnesses

• Injuries

• Asset Damages (Vehicles, Property, Equipment, and Utilities)

• Environmental Events (Releases and Damages)

• Security Issues (Virtual and Physical thefts, breaches, or damages)

• Near Misses (Environmental, General, and Significant)

Reporting Deadlines Include:

• Notifying your Supervisor Immediately 

• Notifying your Safety Rep as soon as possible (not to exceed 8 hrs.)

• Submitting an initial report on EHS Insight within 24 hrs.

Work permits are required prior to the start of work and include the following:

• Confined Space Entry

• Critical Lift

• Extended Hours Request

• Hot Work

• Job Safety Analysis

• Knife Use Permit

• Lockout/Tagout

• Non-critical Lift

• Renewables Dig Permit

• Trenching/Excavation Permit

Any work permit beyond the daily JSA that includes critical tasks will need to be approved by a superintendent or above.
