IMPORTANT NOTICE: The state of New Jersey has executed an executive order (Executive Order No. 252; Executive Order No. 253) to address safety protocols relating to the COVID-19 pandemic. “By October 18th, 2021, all workers in preschool through Grade 12 schools will be required to be fully vaccinated or subject to testing.”
Staff providing services to any families in a New Jersey school must follow this executive order. Should you be placed on a case in the school setting, our team will be in touch to collect your vaccination card OR provide the option to submit for weekly COVID-19 testing.

FBI Clearance
This must be dated within 1 year of submission of paperwork and Behavior Interventions selected as your “employer” using the codes below. Cost is reduced for an archival request for individuals who have previously been printed and as noted on the site for a new print request. These fees are paid for by the employee.

CPR Certification

Completed in part in person (online only courses are NOT accepted)

TB Clearance & Health Appraisal
TB clearances must be dated within two years and kept current (every two years). Health Appraisal is dated within one year and kept current (annually).
It is your responsibility to visit with your doctor and obtain this test and assessment. Behavior Interventions does not cover the cost of this test or office visit. Please, request something in writing from a physician that indicates your test is negative. See TB form for an option form to present your physician. See below on options to obtain this clearance:
You can make an appointment with your primary physician
You can contact your local County Department information for free testing dates
You can walk into many pharmacies without an appointment for this test (ex. CVS, Walgreens & Minute Clinic)
Resume
Resume should meet the following specifications:
Must include your new role with Behavior Interventions
Must be clear and legible
PDF format if sending via email
Include month & year for ALL previous employment dates
Include month & year for ALL education programs
Recommendations:
(see outline for guidance)
Include only relevant experience to the field of applied behavior analysis
Specify related skills, coursework, and work activities specific to applied behavior analysis
Degree
Please include your HIGHEST conferred (completed) degree. Degree should meet the following specifications:
PDF Format if sending electronically: Clear and legible
Include: Your full name, Institution Name, Dates of attendance; INCLUDING graduation date
Acceptable: High School/University Diploma OR conferred college transcripts(unofficial/official).
In addition, please include college transcripts for for any current degrees that you are working toward

Driver’s license should meet the following specifications :

Employment Eligibility Verification
Submit either one item from List A OR furnish one item from from both List B and List C
NOTE: you will complete the official I-9 form electronically via ADP workforce Now. These documents are required to verify your identity per requirements set by designated federal governing entities.
Sexual Misconduct/Child Abuse Disclosure
Completed one form for every previous employer where you have had direct contact with children and received paid compensation (this does NOT include internships or volunteer work) within the past 10 years. These forms will be sent via adobe to your email.

Return form(s) to Behavior Interventions (not your previous employer). We will contact your previous/current employer to complete the remaining sections. Forms sent directly to previous/current employers will NOT be accepted.
Employee Photo
Due to COVID-19, most in-person orientations have been relocated to virtual orientations. Until we are able to bring you into the office, please send us your best headshot to be used for your ID badge, on our company site page and in our welcome email. (Please note, if you do not want to be displayed on the company site please upload your photo and notify your Coordinator that you would like to not be included on the website).
Specifications:
Photo background should be all white (tip: try and locate a white wall or door in your home)
Photo should be professional: smile, take the photo straight on (ie. limit extreme angles), no glasses except prescription lenses should be used, no hats (religious/cultural head coverings are acceptable)
