TB Clearance & Health Appraisal
TB clearances must be dated within two years and kept current (every two years). Health Appraisal is dated within one year and kept current (annually).
It is your responsibility to visit with your doctor and obtain this test and assessment. Behavior Interventions does not cover the cost of this test or office visit. Please, request something in writing from a physician that indicates your test is negative. See TB form for an option form to present your physician. See below on options to obtain this clearance:
You can make an appointment with your primary physician
You can contact your local County Department information for free testing dates
You can walk into many pharmacies without an appointment for this test (ex. CVS, Walgreens & Minute Clinic)
Professional Liability Insurance
Employees can be added to Behavior Interventions' policy at no cost. Please designate your enrollment on your employee checklist if you choose this option.
If you choose to submit a personal policy:
Your plan must include: current coverage dates, policy number, your full name, and specifics of coverage
Specifications: must be for behavioral health services, $1 million individual occurrence & $3 million aggregate.

Resume
Resume should meet the following specifications:
Include Behavior Analyst position at Behavior Interventions
Must be clear and legible
PDF format if sending via email
Include month & year for ALL previous employment dates
Include month & year for ALL education programs
Must document any gaps in employment longer than 6 months (including jobs not related to your profession, volunteering, family leave, etc.)
Recommendations:

Include only relevant experience to the field of applied behavior analysis
Specify related skills, coursework, and work activities specific to applied behavior analysis
Degree
Please include your HIGHEST conferred (completed) degree. Degree should meet the following specifications:
PDF Format if sending electronically: Clear and legible
Include: Your full name, Institution Name, Dates of attendance; INCLUDING graduation date
Acceptable: High School/University Diploma OR conferred college transcripts(unofficial/official).
In addition, please include college transcripts for for any current degrees that you are working toward

Driver’s license should meet the following specifications :
Image must be clear and legible
Include: Date of birth, identification number

Employment Eligibility Verification
Submit either one item from List A OR furnish one item from from both List B and List C
Note: you will complete the official I-9 verification document electronically via ADP workforce now. These documents are required in conjunction with that document to verify your identity per I-9 requirements.
Employee Photo
Due to COVID-19, most in-person orientations have been relocated to virtual orientations. Until we are able to bring you into the office, please send us your best headshot to be used for your ID badge, on our company site page and in our welcome email. (Please note, if you do not want to be displayed on the company site please upload your photo and notify your Coordinator that you would like to not be included on the website).
Specifications:
Photo background should be all white (tip: try and locate a white wall or door in your home)
Photo should be professional: smile, take the photo straight on (ie. limit extreme angles), no glasses except prescription lenses should be used, no hats (religious/cultural head coverings are acceptable)
