Going to your own doctor is a great way to get credit for you Biometric screening. It's best that you and your ACBL medical covered spouse schedule an annual physical ever fall and use the Virgin Pulse Biometric Screening Form below to earn credit.
The best way to submit your form for credit is to upload it in the Virgin Pulse site.
This way you can track your form and it is easier to make sure Virgin Pulse has received it. You can also fax it (the number is on the top of the form) but that is a much slower & less reliable method. The best way to submit your form for credit is to upload it in the Virgin Pulse site.
Instructions on how to upload your forms:
After signing into your Virgin Pulse Wellness Account at ACBLWellness.com click on the green support button on the right-hand side of the screen
Then select Send A Form at the top of the page
Under Please choose your issue below select Submit a Form
Complete form and upload your screening file
Then click Submit

Go to a Quest Lab Facility, appointments are now available on the www.ACBLWellness website.

Log in to www.ACBLWellness.com
Click Benefits
Scroll down and click on Quest Diagnostics
Click START NOW
Click Schedule a Screening
Enter your zip code and click Search
Find the closest Quest Facility to you and click Select
Find the best date & time for you, select it and then press Continue
Then review the information and click Confirm
