• Two networks to choose from:

      • Premier network

• You will pay a little more if you chose a dentist in the Premier network.

• You can see both in-network and out-of-network providers. However, you will pay less if you see a provider who is part of the PPO.

Search for providers

• Dental Health Maintenance Organization (DHMO).

• Coverage applies only to dentists in the Texas service area and are part of the HumanaDental DHMO network.

• You must select a primary care dentist from a list of approved providers.
