School to Work Alliance Program (SWAP)
The School to Work Alliance Program (SWAP) supports young adults with mild to moderate needs in employment.
SWAP helps youth build the skills they need to become competitively employed.
The program focuses on preparing students for successful outcomes in their jobs and communities.

Ensure transition goals focus on employment, education/training, independent living, and community participation, not just school success.
Establish and actively work with a case management agency
Complete applications, update eligibility, and begin planning for adult services that will continue after the student exits the school system.
Support real-world skill building and independence
Encourage your student to practice self-advocacy, manage schedules, participate in work-based learning, and make decisions about their own supports.
Prepare for the transition out of school services
Understand when school supports end, what adult services will replace them, and what steps must be completed before graduation or exit.
