Case Management Agencies help children and adults with disabilities connect to the right supports at the right time. They serve as the “front door” to long-term services, community resources, and developmental disability waivers.
Helps families access disability services and complete eligibility steps
Coordinates assessments and creates annual service plans (ISP)
Connects individuals to community providers, therapies, funding, and resources
Supports long-term planning from childhood through adulthood
