All Laboratory Requisition Forms (LRF) may be obtained from the laboratory technicians in their respective offices. All requests should be made a week before the intended experimentation time.
Each Laboratory Requisition Form (LRF) should be used for a specific transaction:
LRF001 (Within Laboratory Request)
LRF002 (Inter-Laboratory Request)
LRF003 (Experiment Outside Science Period)
LRF004 (Borrowing Equipment for Outside Use)
LRF008 (Usage of Laboratory/Interactive Room)
Breakage Slip - to be filled out in case of breakage. Must be submitted to the laboratory technician before exiting the laboratory.
When making laboratory requests, students must adhere to the following protocols:
WHEN EXPERIMENT/RESEARCH IS TO BE CONDUCTED DURING SCIENCE CLASS TIME
If materials/equipment is/are available inside the assigned laboratory room and would like to borrow the materials/equipment from the laboratory technician, kindly fill out a LABORATORY REQUEST FORM (LRF001).
If material/s/equipment is/are NOT available inside the assigned laboratory room and materials are to be borrowed from the other laboratory room/s, kindly fill out TWO copies of LABORATORY REQUEST FORM (LRF002).
WHEN EXPERIMENT/RESEARCH IS TO BE CONDUCTED BEYOND SCIENCE CLASS TIME AND AT THE CMST LABORATORY ROOM
Visit the laboratory technician/s, present the APPROVED experimental design/methodologies, and inquire on the availability of the materials/equipment/venue. If materials/equipment/venue are available, fill out a LABORATORY REQUEST FORM (LRF003) and have the laboratory technician sign the form.
Submit LRF003 to your Science teacher for endorsement and Science SAC for approval.
Take photos of the approved form and email them to (a) Science teacher (b) Science SAC (jbactad@ateneo.edu) (c) Head Lab Technician (igerona@ateneo,edu) for documentation purposes. EMAIL SUBJECT FORMAT: LAST NAME, FIRST NAME, CN#, CLUSTER & SECTION, LRF#
Surrender the approved form to the laboratory technician AT LEAST THREE DAYS prior to the date of experimentation.
WHEN EXPERIMENT/RESEARCH IS TO BE CONDUCTED OUTSIDE AJHS-CMST LABORATORY ROOMS
Visit the laboratory technician/s, present the APPROVED experimental design/methodologies, and inquire on the availability of the materials/equipment/venue. If materials/equipment/venue are available, fill out a LABORATORY REQUEST FORM (LRF004) and have the laboratory technician sign the form.
Write a letter of request addressed to the Science Subject Area Coordinator (SAC). Make sure that your letter is endorsed by your Science teacher.
Submit the request letter and LRF004 (hard copies) to your Science teacher and/or directly submit it to the Science SAC’s office for approval.
Once approved, take photos of the approved form and email to (a) Science teacher (b) Science SAC (jbactad@ateneo.edu) (c) Head Lab Technician (igerona@ateneo.edu) for documentation purposes. EMAIL SUBJECT FORMAT: LAST NAME, FIRST NAME, CN#___, CLUSTER, GL & SECTION, LRF#___
Surrender hard copies of the approved request letter and LRF004 to the laboratory technician AT LEAST THREE DAYS prior to the date of pick up.
Return the borrowed materials/equipment on the date agreed upon.
In the event that you cannot return on time due to extension of the experimentation period, re-submit a letter of request and again fill out LRF004.
If materials/equipment are NOT returned on time and did NOT re-submit a letter of request & LRF004, The appropriate FORMATIVE INTERVENTION will be given by your Science Teacher.
If materials/equipment are returned but NOT in good condition, borrowed materials/equipment will be charged to your laboratory account and your clearance for the SY will be put on hold until payment is settled.
If materials/equipment are NOT returned, borrowed materials/equipment will be charged to your laboratory account and your clearance for the SY will be put on hold until payment is settled.
