Input up to date School Information
Parent statement signed by one parent or guardian
Employer completes and signs Employer section
The completed work permit application may be submitted in either digital or physical format to Mr. Serna, ROP Teacher, in Room 13, or delivered directly to the Pomona ROP office for review.
