Help with Funerals

Here is an outline of an AHS funeral plan called 'When I move on' which can be adapted and completed with your wishes

Once you have filled in your funeral plan you may want to keep one with you and also send one to Tara - tara@ahs.org.uk in order for it to be stored digitally and be accessible when required.
