Withdrawal

If you are NOT returning to CHMS at any point, you are required to complete a withdrawal form. 

CHMS cannot transfer any files or fulfill any requests from your scholar's new school placement without permissions granted via the withdrawal form. Upon completion, CHMS will officially discharge your scholar so they may be officially enrolled in their new school. 

We cannot discharge your child without the withdrawal form completed.

Once received, CHMS will authorize the discharge to the new school.
