Vision Testing 25-26 SY

Hello CHMS Families, 

Achievement First Crown Heights Middle School will be offering Vision screening to 5th graders, newly transferred students and scholars that have an IEP. The Vision Assessment is to ensure that every student is healthy and ready to learn. Please be advised: the school provided vision screening is strictly to communicate our finding as to your scholar's eye site and should serve only as a basis for recommending that your scholar be taken for further evaluation by their health care provider. 

If you prefer that your scholar, be opt out of vision screening, PLEASE FILL OUT THIS FORM. The deadline to opt out of vision testing is Monday, January 5th. Any opt out form that is not filled out by or before Monday January 5th will result in your scholar being screened. No form filled out will be consent to your scholar being screened as well. Achievement First's mission is that each student has the opportunity to learn and perform at their best. 

We will be testing for the below vision requirements:
-Distance acuity- sharpness of cleanness of vision looking at objects at distance
-Near vision- sharpness of clearness of vision looking at objects close up
