Requesting Transcripts
Purpose
You need to complete a Transcript Request Form for each college to which you are applying. All colleges and universities require an official transcript in order to process an application.

Come to guidance to obtain a Transcript Request Form. This form is our tracking system and lets us know the colleges to which you are applying and their deadlines.
You need to complete a request form for each school to which you are applying. Attached to the request form should be the Secondary School Report/Guidance Form, if applicable. Almost all colleges require a Secondary School Report or Counselor Form, which lists the senior year classes and the counselor’s recommendation. If you are under 18, a parent needs to sign the form.
If your school does not use the Common Application, you need to provide Two Stamped Addressed Business Envelopes with each request form (also affix 2 stamps on each envelope). The second envelope is used to mail your mid-term report, required by each school.
The Transcript Request Form needs to be completed TWO WEEKS prior to the application deadline.
