There is disk height loss at L4-L5 with associated disk dehydration. Notice how the disk has lost its normal water content (not as bright on T2) when compared to the normal levels above and below L4-L5. The disk material has herniated outside of the margins of the annulus fibrosis and is now causing compression of the cauda equina nerve roots.
