Registration for Sage Park Athletics
Before you start, here is what you need:
Signed Medical Provider Statement or Physical Examination Form with Consent for Participation in Athletics (Medical clearance must be obtained every 13 months)
A parent or guardian's electronically-signed permission to participate in interscholastic athletics.
A student-athlete and parent or guardian's electronically-signed version of the WHS' Athletics Code of Ethics agreement.Complete medical clearance paperwork and registration must be submitted to the Athletic Office and approved by the Nurse's Office prior to the first day of practice!
