Immunizations

It is mandatory that you are compliant with the Academic Health Sciences policy regarding required immunizations. Review the forms referenced in the policy.  

It is best to begin the process as soon as possible, as some of the immunizations require time to complete. It may take up to 6 months to complete these requirements.

• Core Immunizations Requirement: Complete the immunization form with a medical provider and submit it to Boynton Health via email to immunizations@umn.edu (forms will only be accepted via email). Please contact Boynton Health to verify they have received your form and that you are compliant. 

• SARS-CoV-2 (COVID-19) Requirement: All University of Minnesota School of Dentistry students must attest to being fully vaccinated against COVID-19. Please complete the following COVID-19 Vaccine Attestation Form in order to self-report your vaccination status. Please complete the form no later than Friday, June 26, 2026. 

• Influenza Requirement: Annual flu shots are required for all Health Science students between August 1 and November 1 of each year. Documentation must be submitted annually to Boynton via z.umn.edu/ahsflushot.
