Employees working 30+ hours per week are eligible for group insurance and may choose to participate in the insurance plans offered. Superior Industries will pay a designated portion of the premium cost for the employee’s health insurance. Benefits include employee’s basic life and basic short-term and long-term disability insurance. Superior Industries will pay a designated portion of the family premium, if family health coverage is desired. Supplemental life and short-term disability insurance are offered at an expense to the employee.
The eligibility of an employee for acceptance into Superior Industries' group insurance plan is determined by the insurance carrier pursuant to rules and policies promulgated by the insurance carrier with respect to the various insurance plans offered. The insurance carrier makes the final decision regarding eligibility or acceptance of an employee into a plan.
The effective date for insurance coverage is the first of the month following completion of the 30-day waiting period. The insured’s insurance will terminate at midnight the last day of the month employed.
In the event of death or termination of employment (including retirement), you or your dependents should be contacted by the personnel department regarding the opportunity to continue health insurance. If you become divorced, or have a child who no longer qualifies as a dependent under the plan, you must notify Human Resources within 60 days of the qualifying event in order to receive continuation rights. You or your dependents will be able to continue health insurance by paying the premium for the maximum time allowed for continuation coverage, which varies according to circumstances (generally from 18 to 36 months). Failure to make premium payments will result in the termination of benefits.
It is important that the employee complete an exit interview with Human Resources upon termination in order to complete paperwork that might be necessary for continuing insurance coverage, if any.
