FAV Liability Agreement Form

Below is a copy of the current FAV Equipment and Key Liability Agreement form every student must sign before they are able to checkout any equipment. This form is avalible to sign in Rebecca Paiva's office, AUD 530, or in FAV Checkout, AUD130.
